I believe its an old rotary phone with an attachment on it so you can hold the receiver more comfortably between your shoulder and head. The angle its at makes it hard to tell.
Definitely a rotary dial phone with a clip attachment (that rests on the shoulder) on the handset that allows the user (eg, a secretary) to conduct a phone call whilst typing.
